The Uttarbasti is then performed in properly sterilised condition. Here, Autoclave oil and instruments are used and the patient is given dorsal lithotomy with the head on low position on the table. The peripheral part and part of vagina are cleaned with betadyne liquid, after which, with the use of Cusco's speculum, the vagina and cervix are visualised. After that, with the help of a uterine, the size and shape of the uterus are determined. The Uttarbasti or IUI cannula which is attached with a 5ml syringe snd filled with medicated oil is then passed into the uterine cavity and the drug is pushed in in such a manner that it can easily reach the fallopian tubes as desired. The patient is then given a head low position for one hour. Generally Uttarbasti with Kshara tail is administered for 3-5 days after menstrual cessation. The procedure is carried out for 3 consecutive menstrual cycles.
The base of the Kshara tail is Tila tail, which is Vyavayi and Sukshma in property, which means that it can spread into minute channels quickly and easily. It is also Snigdha in property, and hence pacifies the Vata dosha. It also has anti-inflammatory results due to its Vranashoshan and Vranapaachan Karma.
